•    Name, age, and address of the requester (applicant)

•    Father's name or spouse's name and details (in which case the applicant is a married woman and is not a permanent resident of the country when applying for a residence permit)

•    The exact period of stay (number of months/years) of the applicant at the specified address.

•    The applicant's declaration stating that the details stated in the affidavit and that all supporting documents are true

•    The purpose for which a domicile certificate is required

•    Applicant's signature

•    The date on which the affidavit was made
